Just FYI: That chip is capable of only delivering 200mA. That is a risk you're taking. It says clearly in the datasheet of the Pico that you need a power supply capable of delivering >=500mA. If you assume you should be good with 200mA if you don't enable WiFi, that decision is entirely on you; you could be correct, you may not be, and if you are correct for one version of the software and SDK, you may not be for another. I'm not saying this for sure is the cause of the problem, but I am saying that you're taking a pretty large risk by deciding that chip 'should be enough for this application'.

Still you may want to test this: can you feed a stable >=500mA capable 3.3V supply (e.g. a lab PSU) straight into the ESP32 and see if it flashes then?
