esp32 buglist - design or manufacturer mistake?

I've been busy for a while with a single question:
maybe you / someone wants to answer someone, for their own sake: esp32 rev0 buglist

were it actually IP design errors or were there manufacturing errors, so production errors?
where was the fault? design / digital department or factory?

For the digital bits at least, they were design errors. Essentially, they snuck into our design when we created it, and our testcases weren't exhaustive enough to uncover them: they mostly are synthetic tests (made to only test one thing) because obviously we didn't have any big integrated apps that would use a lot of the peripherals for the ESP32 yet. While obviously we're always trying to make as few of those errors as possible (and since the ESP32 design, we've actually made some steps in getting better coverage), you unfortunately can't really guarantee there are exactly zero bugs in any silicon.
