ESP32-S3-PICO-1-N8R2 RF power is extremely low

I've just designed a custom board around the ESP32-S3-PICO-1-N8R2. Everything works except that the RF power output is extremely low, approximately -70dB. I believe that everything has been designed properly according to the datasheet. The antenna trace to external antenna is 50 Ohm. The 3.3V power supply is stable. Are there any erratas for the S3-PICO-1 relating to RF power? Or do you have any suggestions for debugging this problem? There doesn't appear to be an eval kit for the ESP32-S3-PICO-1 so I am in the dark.
